Dear Home Care Agencies,
The Department of Health is providing the Attestation Form and Supporting Information to contract with previously non-awarded qualified Lead FIs, who served from January 1, 2020 to March 31, 2020 and were not awarded a contract under #RFO 20039.
NOTE THAT THE ATTESTATION FORM AND SUPPORTING INFORMATION ARE REQUIRED TO BE COMPLETED WITHIN 60 DAYS OF ISSUANCE.
Section 4 of Part PP of Chapter 57 of the Laws of 2022 reviewed Social Services Law Section 365-f to offer previously non-awarded qualified Lead FIs the opportunity to submit an attestation form along with supporting information.
